Member Change Request

I Understand and Accept 
I am the “primary member” listed on my membership agreement and I am authorized to make a change or changes to the membership agreement. I am requesting that my account is updated and modified as outlined in the change form. Nothing will be modified to the original membership agreement except those modifications requested and approved by Prairie Athletic Club. I understand that there are specific fees and conditions required to follow and pay in order for modifications of any kind to be approved. Furthermore, I understand that my request may be denied if they do not meet the requirements and definitions outlined in my membership agreement.

There is no change to my account until accounting and/or sales has reviewed my existing membership. Upon approval or denial of the requested changes, accounting and/or sales will reach out via email or telephone within 3 business days. I further understand that any grandfathered rate will be retired if changes are approved to my membership.

Membership Definition:

• A single membership is designated to a single individual.
• A couple membership is designated to two individuals being billed to one account.
• A family membership is designated to couples or a parent with children under the age of 19 (children ages 19-23 will require an additional fee), who reside in the same home and are being billed to one account. Proof of residency and/or familial relationship satisfactory to Prairie Athletic Club my required for eligibility.
• “Add on” charges may apply to any one in same household or nuclear family being added to a membership.
